Become A Donor
Your decision on whether or not to donate your organs, tissue, and or eyes is a personal decision that is highly respected.
If the decision to donate is made,it is EXTREMELY important to notify your family about your decision. At the time of death, your family must sign a consent form or give verbal confirmation granting Southern Eye Bank permission
to recover ocular tissue at the time of death.
Southern Eye Bank only recovers ocular tissue. For more information on donating organs and tissue, please contact the Louisiana Organ Procurement Agency at the following address:

Signing the Louisiana
State Registry is another way of expressing your wish to donate.
When you renew your driver’s license,
state I.D.or register to vote, please let the Department of Motor
Vehicles (DMV) know that you wish to be registered in the
Louisiana State Registry. It should only take a minute or two to complete
the necessary paperwork. Please inform the clerk if your name has changed so that duplications are removed. By having a heart embossed on your license or I. D., your intentions to donate will be known.
A MYTH ABOUT REGISTRATION...
"If I have a heart on my driver's license, I will not receive adequate care in the hospital because my organs, tissue and eyes are needed."
THE FACTS ARE...
EVERY effort is made by the hospital team to save your life whether you have a heart on your driver's license or not. Only AFTER you are declared deceased will Southern Eye Bank be notified of your death. Your family will be approached by a hospital liaison a short time after your death about your intentions to donate. PLEASE LET YOUR FAMILY KNOW ABOUT YOUR DECISION
TO DONATE! Your Next of Kin will ultimately be your deciding factor.
