1. Call our administrative assistant with the following information:

        Patient name, unique identifying number, sex, race, date of birth and address. Also state the performing surgeon, name of surgery center or hospital (please include address of surgical institution). Diagnosis and eye needing surgery is also required.

Please inform us if the tissue needed is for a pre-cut Endothelial Keratoplasty or a Penetrating Keratoplasty Procedure.

2. For local surgeries (within the United States) our technicians will call the day before the scheduled surgery    and give the surgery coordinator (or surgeon) the specifications of the tissue.

For international tissue distribution or Monday morning procedures, our technicians will call in advance to discuss shipping arrangements.

3. Surgery Centers / Hospitals will be invoiced within 7-10 days post receipt of tissue.

For your convenience, we can e-mail invoices. for international and intranational surgeries.

Payment is due within 30 days of the invoice date.

4.   Our staff members are always appreciative of post-operative success stories! If you have a patient who would like to share his / her story, please contact us with the information  and we will share their story with future recipients who are anticipating their surgery and also with donor families who have graciously and selflessly donated their loved one's ocular  tissue.
