HOW TO ORDER TISSUE FROM               

                                  SOUTHERN EYE BANK

     1. Call our administrative assistant with the following information:

        Patient name, sex, race, date of birth and address

        Performing surgeon, name of surgery center or hospital (please include address of surgical institution)

        Diagnosis and eye needing surgery.

 2. For local surgeries (within the United States), our technicians will call the day before the

      scheduled surgery and give the surgery coordinator (or surgeon) the specifications of the tissue.

     For surgery to be performed on Monday morning, our technicians will call on Thursday unless

     prior arrangements have been made between our technicians and the coordinator or surgeon.

     For international tissue distribution, our technicians will call in advance to discuss shipping            arrangements.

                                   3. Surgery center or hospital will be invoiced within 7-10 days post receipt of tissue. 

     For your convenience, we can e-mail invoices. For international                

      surgeries, invoices will be emailed. Payment is due within 30 days of the 

     invoice date.  

                                 4.    If you have a patient who would like to share their story,

           please contact us with the information and we will share their story with future recipients who are                anticipating their surgery and with donor families who have graciously and selflessly donated their

           loved ones ocular tissue. Your patients are able to write their donor's family if they
